Date Selection Convenience for Portfolio/Fund Report Form

[Delivered via DECAF Cafelatte Web Application v1.4.0]

Users can now select the date either from the date-picker or from one of the pre-defined dates provided as a dropdown.

These pre-defined dates now includes the inception date of the portfolio/fund, too.
